Large Home Health player in Dallas is looking for a PT's, with interest in Home Care. This position plans, interprets and implements specific treatment programs for individual patients. Treatments and care may involve in-patients, home health care patients and outside contract services. May also be directed by Home Health Clinical Supervisor. Primary Job Functions: · Plans and evaluates patients' treatment programs in coordination with medical staff. Administers and clarifies treatment program per physician instructions. Recommends solutions to patient care problems. Discusses and confers with interdisciplinary treatments teams on patient care and treatment plans. · Educates patients and family in proper care and use of various supports and exercise programs. Follows up with patients during treatment, explaining precautions to take. · Evaluates charts and reports on patients' progress. Maintains patients' records, charts, and discharge summaries. · Initiates charges to patients according to established procedures. · Recommends physical therapy policies and procedures concerning services provided. Assists other therapists in planning treatment programs. · Participates in performance improvement program, safety, environmental and infection control. Assists with data collection and prepares reports. Maintains patient statistics. · Conducts and participates in clinic educational programs. · Performs other functions as required. Ideal candidate will have strong interest and experience in home care.
